Turn it on, normal playing loudness, without any media playing. If you hear anything: hum, hiss, buzz, there's a problem. There is no excuse for background noise in a modern system. The problem could be bad design, defective hardware, poor installation, bad interconnects, ground loops, bad power wiring, etc.
What are your red flags?

From what distance? Normal listing distance (~10 ft or more), absolutely. Ear one inch from a horn, might be a little hiss......

What are your red flags? Sincerely /b Yes, but loose or defective interconnects, ground loops, bad power wiring, etc. is very different than poor design, cheap components, defective hardware, etc. Two separate catagories. One can be a "bad" system while the other is "bad" installation.

As I understand it a bad system is where flaws would be universal. Things that are not subject to taste or preferences. There are a lot of systems I won't like out there, that someone else will like! So a bad system is one with defects that will disrupt the listening experience (or safety) of the operator!
Any audible sound not coming from the original record qualifies, as well as cruel dis-balance caused by various sources of peaks and their opposites. Room modes, bad post processing, inadequate combinations...
Now what is a good system... something else entirely!

This one's easy. There are several ways to identify a bad system: #1 - A bad system includes components not fully endorsed by 6moons (or was it Stereophile? I keep forgetting the difference between the two). #2 - When comparing two different systems, the bad system is always the less expensive of the two (e.g. a $100K system is worse than a $101K system). #3 - If all else fails, a bad system is the one owned by some Kool-Aid drinking fanboi on the Emotiva Lounge, because you are a member of the esteemed [redacted]. Next? Disclaimer: the above is tongue in cheek.

A bad system, for me, is one where I can't tell what color lipstick the singer's wearing..... I prefer listening to guys who don't wear lipstick ..... just a personal preference.

And guess what it doesn't have to he modern to have hiss, buzz, hum, etc. Those have been, and will always be around if poor install products, or practices are used.

How to tell a bad system? Well I think you just sit it down and have a frank discussion, often it's better to simply tell the truth. "I'm sorry, but you're just a bad system" Insert distracting picture here ................. Cheers Gary
